If you are interested in working together, please start by filling out the form below. You will then be re-directed to a calendar portal to schedule your free 20-minute consultation call, followed by an email confirmation with your appointment date and time. On our call, I will answer any questions you may have to determine if I can meet your needs.
If you have any other questions, other than scheduling, please contact me here. If you are unable to find a time that works for you on the scheduling page, please let me know and I can likely accommodate your specific needs outside of traditional business hours.
Prior to scheduling please note the following:
1) I am licensed in Washington state only, meaning that I can only see patients who are physically located in WA at the time of our visits.
2) All appointments are conducted via remote video sessions (telehealth) at this time. I may offer in-person visits in the future at my clinical discretion.
3) I do not accept insurance or negotiate with insurance companies. If you have out-of-network benefits, I will provide a super bill (at your request), which you can submit to your insurance for potential partial reimbursement. However, I cannot guarantee your insurance company will reimburse you for my fees. Please review my fees here (bottom of page) prior to scheduling.
4) I do not offer sliding scales or discounted rates.
